A weight-loss questionnaire is a set of questions that a person can answer in order to determine their current body mass index. For those of you who are already familiar with how much weight you have gained, please read each question carefully.

This questionnaire is designed to help people identify their true personality type. It is important that you answer all the questions honestly and truthfully as this will help your weight loss journey so that you get the right type of treatment.
